The Medical Information Notebook Program is a 2-part educational series that covers topics such as family history, screenings, immunizations, medication storage and disposal, generic medications, side effects of medications, advance directives, hospice, organ donation and funeral planning. Participants will receive an organizational binder to complete and take home.

The program includes presentations from Timothy Mummert, DO, on medical history, screenings, immunizations and medical tests; one of our Magruder Pharmacists on medication storage, generic medications, side effects, herbals and more; Hospice of the Western Reserve on advance directives and end of life issues; Solvita on organ donation; and Walker Funeral Homes on funeral planning.

Lunch will be provided both days.

Participants will receive an organizational binder to complete and take home.
