Community Health Worker
Thanks to a grant from the Northwest Ohio Pathways Hub designed to support residents of Ottawa County, Magruder now has a Certified Community Health Worker.
The Magruder Community Health Worker can help connect clients of any age to resources for any of the following referral risk factors:
- alcohol/substance abuse
- childcare
- chronic disease (heart, diabetes etc.)
- depression or other mental health needs
- developmental delays
- domestic violence
- education assistance
- employment
- financial assistance
- food
- housing
- insurance
- legal
- nutrition and physical activity
- transportation
- …and more.
How does the referral process work?
Anyone who identifies an at-risk community member can reach out to the Magruder Community Health Worker.
The Community Health Worker will reach out to that community member to talk over what their needs and barriers are.
The Community Health Worker connects the community member to resources to help meet their needs and minimize any barriers.
The Community Health Worker stays connected to the community member to make sure other needs or barriers are addressed.
This approach helps community members become self-sufficient while also addressing the social determinants of health, reducing disparities and improving population health.
Anyone can refer a community member in need to the CHW program, and the Community Health Worker will meet with them to assess their needs and come up with a plan to overcome barriers they might have.
