Primary Care

Primary care is the hub of your healthcare wheel; it is your healthcare home. And your Primary Care Provider (PCP) is the one who helps you prevent illness, as well as diagnose and treat conditions you may have throughout your life. To help achieve your full potential for health and well-being, you want a PCP who knows and understands your health history, your health goals and your concerns.

A Primary Care Provider can be a physician or a nurse practitioner. And they may be trained in:

Family Medicine – Providers care for people of all ages, from infants through elderly – or

Internal Medicine – Providers care for adults, generally age 18+

Your PCP will be there to provide:

  • preventive services such as vaccinations, screenings or lifestyle coaching
  • care for injuries and illnesses as they come up
  • help as you manage chronic or long-term conditions like diabetes or asthma

Because a PCP works with you throughout your lifespan, they can regularly monitor your health, track changes and help you decide if you need to see a specialist.
